Patient Portal
Welcome to the patient portal. Once you click “Activate Account” and sign up, you will be able to access your information including:
- Purchase history and receipts
- See your balances
- Retrieve your prescriptions for glasses and contacts
- View your appointments
- Validate your personal information
A valid email address is required to sign up at the patient portal. You should use the same email address that you have provided to the practice.

If you cannot activate or access your account, call your office for assistance. Do not send passwords or sensitive medical information through a general website contact form.
